Looking for the right place to recover from surgery or illness?

When you find yourself between hospital-level care and returning to your home and work, The Transitional Care Center at Kirkhaven is the perfect place to regain your health and get back on your feet.

At Kirkhaven, we're small enough to render personalized care, and yet we're fully-equipped to offer a variety of specialized services. We are actively expanding the conditions and levels of acuity for which we can provide care. Our Transitional Care Center capabilities help meet the demand for more affordable health options, especially for those who do not need or desire to be in a hospital.

Personalized Care Plan

Our goal is to help our rehab clients become as independent as possible...as quickly as possible. We start with an individualized care plan developed by a multi-disciplinary team; everyone from your physician to the nurses to the therapists and nutritionists - even the housekeeping department gets involved. Communication is key to achieving optimal rehabilitation results, which is why everyone who affects your care at Kirkhaven is kept in the loop. The care team meets weekly to assess and revise your care plan, to keep your recovery and discharge goals on-track.

Many nursing facilities do not have weekend therapy sessions, which can mean a delay in your progress. At Kirkhaven, we work on Saturdays. We also provide one-hour sessions instead of the standard 30 minutes. This allows plenty of time for discussion and patient education. We want you to feel comfortable and fully competent in your own care before its time to go home. Our therapists can also evaluate your home for your continuing recovery needs, and set-up additional home services and out-patient therapy if necessary.
